This corresponds to a metatarsalgia (see), occurring mainly between the 2nd and 3rd or the 3rd and 4th toes, where the plantar digital nerve divides. Compression of the nerve at this level by the presence of a type of “mass” called a neuroma is the cause of the symptoms. The presence of greater mobility in one intermetatarsal space compared to the adjacent space favors the occurrence of microtrauma in this area and may be the origin of the problem. It may also be related to alterations in foot biomechanics and the use of inappropriate footwear. Symptoms include intense pain in the region, aggravated by wearing tight shoes, weight-bearing while standing, walking, tingling sensations in the forefoot, and electric shock-like sensations.
